Survey must be completed in its entirety. One valid entry allowed per customer account, per order placed. Employees of Polymer Molding, Inc. and their relatives are not eligible. A random drawing of qualified participants will take place on December 31, 2017 and the winner will receive a Fitbit. Shipping Charges will be at the expense of Polymer Molding, Inc.
